Hypertensive Male with Poorly Controlled T2DM and Swelling

 A 42-year-old man with a history of hypertension and poorly controlled type-2 diabetes mellitus complicated by end-stage renal disease (ESRD) presented with acute right lower extremity pain and swelling. Laboratory values showed slightly elevated D-dimer of 1.52 μ/mL, elevated C- reactive protein of 8.626 mg/dL, elevated erythrocyte sedimentation rate, and elevated creatinine kinase of 381 U/L.  

Magnetic resonance imaging (MRI) of the right lower extremity demonstrated enlargement of the right thigh, and extensive subcutaneous edema and fascial fluid were also noted (
